Antimicrobial Stewardship

COMBAT-AMR will provide training and mentorship to build capacity for improved measurement and ongoing monitoring of the appropriateness of antibiotic use, as well as the capability to manage antimicrobial stewardship and respond to emerging issues.

This theme will be implemented across all four program countries, in partnership with the Fleming Fund Fellowships and Country Grant in Papua New Guinea, with the aim to:

Improve measurement of the appropriateness of antibiotic use in hospitals and monitoring of volumes of use.

The readiness assessment will examine the current policies, practices and guidelines that may impact on antimicrobial use. Implementation of audit tools will be used to measure appropriateness of antimicrobial use against existing guidelines. This will help to describe patterns of use and identify target areas for action. Ongoing training will be provided to help establish a program of regular auditing of antimicrobial use and to support reporting of audit results for meaningful action.

Improve capacity to manage antimicrobial stewardship.

Capacity to manage Antimicrobial Stewardship (AMS) will be assessed at each institution and opportunities to build AMS knowledge and skills amongst varied healthcare staff will be explored through workshops and training activities. Support for the implementation of AMS programs includes the adaptation of resources, assistance to review and update local guidelines for antibiotic use, communication and awareness programs amongst staff and consumers, and support to implement strategies to improve AMS more generally across the healthcare settings.

Establish capacity to respond to emerging issues (e.g. outbreaks) with AMS and IPC.

The IPC and AMS components of this program will come together to discuss basic responses to outbreaks of AMR pathogens of public health significance. Planning for IPC efforts to limit transmission, and AMS activity to ensure appropriate treatment of the pathogen where possible will be explored (e.g. responsive guidelines).

Infection Prevention and Control

COMBAT-AMR will improve Infection Prevention and Control (IPC) by establishing best practice policies, procedures and systems for monitoring selected healthcare associated infections through training and mentoring.

Laboratory Capacity and Surveillance

COMBAT-AMR will improve capacity for laboratory surveillance of AMR through training and mentoring activities to detect AMR in an accurate and timely manner, to undertake surveillance of WHO AMR priority pathogens, and to establish efficient referral pathways for AMR pathogens.

COMBAT-AMR will help mitigate the threat of antimicrobial resistance arising using a One Health approach, by assessing current practices, policies, procedures and systems for monitoring and controlling antimicrobial resistance in animal health and developing approaches to address needs through training and mentoring.
